Majma al-Zawaid — Hadith 3664

Sayyiduna Abdullah bin Amr (may Allah be pleased with him) narrates that the Prophet (peace and blessings be upon him) said: "Whoever performs ablution and does it well, then prays four rak'ahs in which he does not fall into heedlessness, he is forgiven."
Al-Bazzar recorded it. Its chain contains a narrator, Abd al-Karim bin Abi al-Mukhariq, and he is da'if (weak).
