It is narrated from Sayyiduna Ibn Abbas (may Allah be pleased with them both) that the Noble Prophet (peace and blessings be upon him) had cupping done while in the state of ihram. Ishaq Rahwayh said: Sufyan narrated once from Ata, once from Tawus, and once from both of them.

Brief Explanation
(Commentary on Hadith 1858)
In light of these authentic narrations, the esteemed scholars have reached consensus (ijma‘) on the permissibility of cupping (hijama) while in the state of ihram, whether it is performed on the head or any other part of the body, whether there is a need or not. The condition is that hair should not have to be cut. If hair is broken or has to be cut, then offering a compensatory sacrifice (fidyah/dam) will become obligatory. From this, it is also understood that if, due to necessity, the head has to be shaved, or stitched clothing has to be worn, or game is hunted and killed, then in all these matters, fidyah (dam) becomes obligatory upon the person in ihram.
