It is permissible for a person in the state of Ihram to apply kohl (surma) or put any medicinal drops/ointment in the eyes for treatment purposes.
This is proven from the narration of Hazrat Uthman رضي الله عنه:
The Messenger of Allah ﷺ said regarding a person in Ihram whose eyes are hurting:
"ضمدها بالصبر"
“Apply aloe (a type of medicine) to them.”
(Sahih Muslim: 2089, Kitab al-Hajj, Bab: The permissibility for a Muhrim to medicate his eyes)
